Clinical Experience Introduction of Classic Formular of Bu Yang Huan Wu Tang in Treating Palpitation

Wang Qingren, an ancient Chinese Medicine practitioner in the Qing Dynasty, developed the method of “boosting qi for dissolving blood stasis”, which has been used and honored by generation after generation. The representative formula is Bu Yang Huan Wu Tang, from his masterpieceCorrection of Errors in Medical Works.It composes of Huang Qi, Dang Gui, Chi Shao, Di Long, ChuanXiong, HongHua and Tao Ren.

Wang believed that “Once original qi is deficient, it is impossible to reach blood vessels. Therefore, qi in theblood vessels is insufficient,and blood circulation becomes unsmooth, leading to blood stasis.” So, Bu Yang Huan Wu Tangincludes a large amount of Huang Qi to greatly reinforce spleen qi and lung qi.Together with Dang Gui, Chi Shao, Di Long, Chuan Xiong, Tao Ren and Hong Hua, it can activate blood movement. The formula has been applied thousands of times by later generation TCM practitioners and promoted as the most important formula to treat the sequelae of stroke.Wang divided yang qi of the human body into 10 shares, which are distributed all over the body, and each side of the body gets 5 shares of it. If the human body loses 5 shares of yang qi, there areonly5 shares left, which will cause qi deficiency, resulting in hemiplegia. Therefore, Wang created this formula to boost qi, invigorate blood, dissolve stasis andunblock collateralsto "recover the lost half".In this way, yang qi can be restored and the hemiplegia can be relieved,and that's why the formula is called "Bu Yang Huan Wu Tang(means to boost yang qito regain the other 5 shares)".

Application of Bu Yang Huan Wu Tang has 3 characters.Firstly, it containsHuang Qi as the chief medicinal.The quantity is large and the effect is focusing, whichnot only promotes sourcetransformation of spleen and stomach, but also protects genuineqi of meridians and collaterals, so as to make sufficientYing qi and Wei qito invigorate blood vessels. Secondly, the dosages of herbsfor activating blood circulation and removing collateral obstruction are small, so that the whole formula removes blood stasis but not harming healthy qi. Thirdly, the dosage ofHuang Qi is not only large, but also increased gradually. Especially for those with severe qi deficiency, a large amount of Huang Qi is required tobuild up healthy Qi.Bu Yang Huan Wu Tang was originally used to treat the sequelae of stroke, such as hemiplegia, deviated mouth and eyes, dysphasia, etc. But in clinical practice, this formula is also effective for dizziness, palpitation, chest oppression, body pain and other medical disorders that caused by qi deficiency and blood stasis according to TCM principles and methods. Xu, female, 45-year-old, Nov. 11th, 2018

Xu had recurrent palpitation for over 10 months.She complainedof palpitation from time to time, short of breath, lazy to speak, thirsty with desire to drink, reversal cold of fingers and easily getting fatigued. Her appetite, sleep, urination and defecation were all normal. The pulse was thin and weak. She had a pale-large and teeth-marked tongue with thin white coating. She had a medical history of arrhythmia and hypotension. BP 80/60mmHg.

Western diagnosis: Arrhythmia.

TCM diagnosis: Palpitation (qi deficiency and blood stasis).

ModifiedBu Yang Huan Wu Tang was used here to boost qi and activate blood.

Huang Qi 45g, Dang Gui 12g, Chuan Xiong 10g, Chi Shao 12g, Tao Ren 10g, Hong Hua 6g, Dang Shen 20g, Bai Zhu 12g, Fu Ling 12g, Zhi Gan Cao 9g, Chen Pi 9g andSan Qi 6g, 7 dosages.

After taking the formula for 7 days, the symptoms of palpitation, short of breath and lazy to speak were all improved obviously. Lowspirit and dry mouth were also improved, yet she still had reversal cold of fingers.Her appetite, sleep, urination and defecationwere all normal. Herpulse was thin and weak. She had a pale-large tongue with teethmarks on the margin and thin white coating. Based on the previous formula, Rou Gui 3g was removed.The method of boosting qi and activating blood continued to be applied for 7 more days.

After taking the formula for 7 days, the symptom of dry mouth was cured.Occasionally she hadpalpitation, short of breath and lazy to speak.The low-spirit was improved and her fingers became warmer.Her appetite, sleep, urination and defecation were all good. The pulse was thin and weak. Shehad a pale-large tongue with teethmarks on the margin and thin white coating. Again, the last formula continued to be used to boost qi and activate blood for 7 days.

Notes: The patient suffered from palpitation for over 10 months. Shehad visited many other doctors, but not found the solution. When she came to the TCM clinic, she complained about palpitation, short of breath, lazy to speak, fatigue andreversal cold of hands. She hadthin-weak pulse, and a pale-large tongue with teethmarks on the margin and thin white coating, which webelievedwas due to qi insufficiency, failingin promoting blood circulation. So, we used modifiedBu Yang Huan Wu Tang to improve her symptoms. Huang Qi is sweet and warm,which is the key herb for boosting qi. Therefore, a large amount of Huang Qi was applied. “Qi is the commander ofblood, and qi movement promotes bloodcirculation.” In this formula, Tao Hong Si Wu Tang (with Sheng Di Huang removed) was used to nourish and invigorate blood. And because this patient had a pale tongue and her body constitution was cold, salty-cold Di Long was removed and bitter-warm San Qi was added to invigorate blood and dissolve stasis. Also, Yi Gong San was added to help Huang Qi reinforce original qi. On the patient's second visit, we added RouGui not only to inspire the growth of qi and blood, but also to warm channels andinvigorate blood. All those herbs in one formulawere applied to boost qi and invigorate blood, as well as dissolve stasis andunblock collaterals, thus to resolve her symptoms gradually.
